And no, there isn't systematically somebody in the cockpit from what I read.

I've been involved in aircraft maintenance for a very long time & there is a shear pin in the towing arm which will break if say the aircraft brakes are on,to save damage to the nose leg.

For that reason there should always be someone riding brakes should the shear pin go.I've done it & been there when it's happened.

Either there was nobody on brakes,there was someone & he/she was asleep or playing with their smartphone or there was no hydraulic pressure to the brakes.
