During the initial development of the foetus it is androgynous, some say initially female. This is when nipples are developed. Sex organs are a later development. While we are taught that there are X and Y chromosomes, the system is not perfect and some the dangly bits that are called X and Y are not fully or consistently developed giving a range of options that gives the many people with limited education a headache. Genetics is not a precise science, otherwise there would be no variation.

Very nice young lady who used her work proceeds to pay for college. Her boyfriend at the time got her interested in parachuting.... On her solo jump the bindings on the parachute jerked her nipple real hard. She had forgotten to take out the nipple ring. Went down that jump in agony. 1
