Medical (not end of life) care is different (and probably education too) as scaling things up in size while having a centralised structure gives benefits which are measurable and quantifiable. However, I would agree that this is not the case with supplying food, houses, or end of life care where local (family) providers can supply this if both the giver and receiver have planned accordingly.
