A lack of proper piloting can be attributed to a lot of major incidents.

Indonesia has an awful aviation safety record.

No amount of maintenance can make the thing idiot proof methinks!

Even if they manage, it won't be proof against the next generation of improved idiots.

In the UK we have duplicate or independent inspections.

If any work is carried out on anything that effects the control and handling of the aircraft it,as all work carried out,has to be countersigned by an appropriately licenced and approved engineer,then inspected and signed for by another lic & app'd engineer who has not been involved in the work.That applies to functional checks as well.

Unfortunately some countries including some in EASA do not have these stipulations and/or pay scant regard to them.

Even the fitment of passenger seats is usually a duplicate inspection.
